EXPORT END USER FORM (D4-SG07) INSTRUCTIONS
Key definitions used in these instructions:
The term "Assembly" means the module or device into which the Mini-Circuits part will be directly installed or integrated. For example, if the Mini-Circuits part is incorporated into a navigation system that will ultimately be installed in an automobile, the navigation system is the Assembly.
The term "Controlled End Use" means any of the end uses referred to in B(1), B(2), and B(3) of BLOCK 4.
The term "End User" means (i) the company using the parts or equipment for lab use; (ii) the company or institution using the parts or equipment for research, development, or educational purposes; or (iii) the company incorporating parts into its Assembly.
The term "Ultimate End Item" means a system, equipment, or assembled commodity ready for its intended use. Only ammunition, fuel or other energy source is required to place it in an operating state for its intended use. Examples of Ultimate End Items include automobiles, ships, aircraft, and firearms.
Instructions for completing Export End User Form D4-SG07:
BLOCK 1- Name of Customer Submitting this Form: Provide the full legal name of your company.
BLOCK 2- Company Name of End User: Provide the company name of the End User.
BLOCK 3- End User Address: Provide the address of the End User. If you provided a company name in BLOCK 2, the complete address for that company should be provided in BLOCK 3, otherwise provide the complete address of your company. The complete address includes the street address, the city, the state/province, the zip/postal code, and the country.
BLOCK 4- End Use: If the parts are:- used in any way for a nuclear activity (including a maritime nuclear propulsion project), check box B and box (1) for "used in a nuclear activity."
- used for lab use in any of the Controlled End Uses, check box B and the box for the appropriate Controlled End Uses ((1), (2), or (3)).
- used for lab use but NOT in any of the Controlled End Uses, check box A.
- used for research, development, or educational purposes in any of the Controlled End Uses, check box B and the box for the appropriate Controlled End Uses ((1), (2), or (3)).
- used for research, development, or educational purposes but NOT in any of the Controlled End Uses, check box A.
- incorporated in an Assembly (not Ultimate End Items) that is any of the Controlled End Uses, check box B and the box for the appropriate Controlled End Uses ((1), (2), or (3)).
- incorporated in an Assembly (not Ultimate End Items) that is NOT any of the Controlled End Uses, check box A. However, if your Assembly is incorporated into an Ultimate End Item used in a nuclear activity, check box B and the "used in a nuclear activity" box (1).
